Plaque Psoriasis
Plaque
psoriasis is the most common form of the disease and appears as raised, red
patches covered with a silvery white buildup of dead skin cells or scale. These
patches or plaques most often appear on the scalp, knees, elbows, and lower
back. They are often itchy and painful, and they can crack and bleed.
Symptoms
Raised
red scales with well-defined edges, Plaque psoriasis is most often found on the
outside of knees and elbows, the scalp, the lower back, the face, the palms and
soles of feet. Psoriasis skin looks thicker and inflamed when compared with
eczema.
Treatment
Keep
in mind, no single psoriasis treatment works for everyone. Working closely with
a specialist is key to bringing psoriasis into remission. Symptomatic

Guttate Psoriasis
Guttate
[GUH-tate] psoriasis is a form of psoriasis that often starts in childhood or
young adulthood. This is the second most common type of psoriasis, after plaque
psoriasis. About 10 percent of people who get psoriasis develop guttate
psoriasis.
Symptoms
This
form of psoriasis appears as small, red, separate spots on the skin. Guttate
lesions usually appear on the trunk and limbs and can number in the hundreds.
Sometimes lesions form on the scalp, face and ears. They usually are not as
thick as the lesions that characterize plaque psoriasis. This form can precede
or co-exist with other forms of psoriasis, such as plaque.
Triggering factors
Guttate
psoriasis often comes on quite suddenly. There are a variety of known triggers,
including:
¬ Upper respiratory infections
¬ Streptococcal infections
¬ Tonsillitis
¬ Stress
¬ Injury to the skin
¬ Certain drugs (including antimalarials and beta blockers)
A
streptococcal infection of the throat (strep throat) is a common guttate
psoriasis trigger. You can have strep throat without symptoms. If you have a
flare of guttate psoriasis contact the specialist without making delay.

Inverse Psoriasis
Inverse
psoriasis also known as inter triginous psoriasis, shows up as very red lesions
in body folds. It may appear smooth and shiny. Many people have another type of
psoriasis elsewhere on the body at the same time.
Symptoms
Inverse
psoriasis is found in the armpits, groin, under the breasts and in other skin
folds on the body. It is particularly subject to irritation from rubbing and
sweating because of its location in skin folds and tender areas. It usually
lacks the scale associated with plaque psoriasis due to the moist environment.
It is more common in overweight people and people with deep skin folds.

Pustular Psoriasis
Pustular [PUHS-choo-lar] psoriasis in
characterized by white pustules (blisters of noninfectious pus) surrounded by
red skin. The pus consists of white blood cells. It is not an infection, nor is
it contagious.
Symptoms
Pustular
psoriasis is primarily seen in adults. It may be limited to certain areas of
the body, for example, the hands and feet. Generalized pustular psoriasis also
can cover most of the body. It tends to go in a cycle with reddening of the
skin followed by pustules and scaling.
Triggering factors
Many factors
may trigger pustular psoriasis, including:
¬ Internal medications
¬ Irritating topical agents
¬ Overexposure to UV light
¬ Pregnancy
¬ Systemic steroids
¬ Infections
¬ Emotional stress
¬ Sudden withdrawal of systemic medications or potent topical
steroids
Types of Pustular Psoriasis
v Von Zumbusch can appear abruptly on the skin. It is characterized by
widespread areas of reddened skin, which become painful and tender. Within
hours, the pustules appear. Over the next 24 to 48 hours, the pustules dry,
leaving the skin with a glazed and smooth appearance. Children rarely develop
Von Zumbusch pustular psoriasis, but when it does happen it is often the first
psoriasis flare and may have a better outcome than in adults. This form can be
life-threatening and requires immediate medical care. People with von Zumbusch
pustular psoriasis often need to be hospitalized for rehydration and start
topical and systemic treatment, which typically includes antibiotics. Von
Zumbusch is associated with fever, chills, severe itching, dehydration, a rapid
pulse rate, exhaustion, anemia, weight loss and muscle weakness.
Palmo Plantar pustulosis (PPP) causes pustules on the palms of the hand and soles of the feet.
It commonly affects the base of the thumb and the sides of the heels. Pustules
initially appear in a studded pattern on top of red plaques of skin, but then
turn brown, peel and become crusted. PPP is usually cyclical, with new crops of
pustules followed by periods of low activity.
Acropustulosis (acro dermatitis continua
of Hallopeau) is a rare type of
psoriasis characterized by skin lesions on the ends of the fingers and
sometimes on the toes. The eruption occasionally starts after an injury to the
skin or infection. The lesions can be painful and disabling, and cause
deformity of the nails. Occasionally bone changes occur in severe cases.
Treatment
Treatments
for specific types of pustular psoriasis include:
Ø Generalized
pustular psoriasis: The goal
of treatment is to prevent infection and fluid loss, stabilize the body's
temperature and restore the skin's chemical balance.
Ø Localized
pustular psoriasis: This form
can be stubborn to treat. Topical treatments are usually prescribed first.
Ø Von Zumbusch: Symptomatic Treatment given by doctors.
Ø Palmoplantar
pustulosis: Because PP often is
stubborn to treat, doctors usually prescribe topical treatments first, and then
consider other options,
Ø Acropustulosis: Traditionally this form of pustular psoriasis has been hard to
treat. Occlusion of topical preparations may help some people. Some people have
had success using systemic medications to clear lesions and restore the nails.
Erythrodermic Psoriasis
Erythrodermic
[eh-REETH-ro-der-mik] psoriasis is a particularly inflammatory form of
psoriasis that often affects most of the body surface. It may occur in
association with von Zumbusch pustular psoriasis. It is a rare type of
psoriasis, occurring once or more during the lifetime of 3 percent of people
who have psoriasis.
It
generally appears on people who have unstable plaque psoriasis. This means the
lesions are not clearly defined. Widespread, fiery redness and exfoliation of
the skin characterize this form. Severe itching and pain often accompanies it.
Individuals
having an erythrodermic psoriasis flare should see a doctor immediately. This form of
psoriasis can be life-threatening.
Symptoms
v Severe redness and shedding of skin over a large area of the
body
v Exfoliation often occurs in large "sheets" instead of
smaller scales
v Skin looks as if it has been burned
v Heart rate increases
v Severe itching and pain
v Body temperature goes up and down, especially on very hot or
cold days
Erythrodermic
psoriasis "throws off" the body's chemistry. This causes protein and
fluid loss that can lead to severe illness. Edema - swelling from fluid
retention, especially around the ankles, may develop, along with infection. The
body may not be able to maintain its temperature. This can produce shivering
episodes.
Erythrodermic
psoriasis also can bring on pneumonia and congestive heart failure. People with
severe cases often require hospitalization. Erythrodermic psoriasis can occur
abruptly at the first signs of psoriasis or it can come on gradually in people
with plaque psoriasis. The reason erythrodermic psoriasis appears is not
understood. However, there are some known triggers.
Triggering factors
¬ Abrupt withdrawal of systemic treatment
¬ Severe sunburn
¬ Allergic, drug-induced rash that brings on the Koebner
phenomenon (a tendency for psoriasis to appear on the site of skin injuries)
¬ Use of systemic steroids (cortisone)
¬ Infection
¬ Emotional stress
¬ Alcoholism
Treatment
Symptomatic
medications are most effective and usually are required to bring severe cases
under control.
